About Types of Care
What is Transitional Care?
Transitional care refers to the support provided when an individual is moving from a hospital or rehabilitation facility back to their home. This type of care is essential for a smooth recovery, whether after a procedure or hospital stay. We offer personalized transitional care plans, which include transportation from the hospital or facility, assistance with follow-up appointments, and support during recovery.
During the transition period, patients may face both physical challenges and emotional hurdles, such as anxiety or depression from not being able to engage in normal activities. Our caregivers help ease this process by providing companionship, assisting with physician-prescribed exercises, and helping clients stay connected with family and friends through technology. This comprehensive support system promotes both physical and emotional well-being during recovery.
What is Companion Care?
Companion care focuses on providing non-medical assistance and emotional support for seniors. Our companion care services include meal preparation, light housekeeping, grocery shopping, and transportation to appointments or social events. We also offer reminders for grooming and medication, and keep families updated on their loved one’s well-being.
In addition to helping with daily tasks, we prioritize activities that bring joy and mental stimulation. By fostering meaningful connections, we aim to enhance our clients’ emotional well-being, ensuring they stay engaged and active.
What is Personal Care?
Our personal care involves providing hands-on assistance with daily activities, such as bathing, mobility support, incontinence care, and meal preparation. Our trained caregivers offer varying levels of assistance, from stand-by help to full care, depending on each client’s needs.
For individuals with cognitive impairments, we provide specialized care, including fall risk assessments and assistance with outings or social events. Personal care goes beyond basic tasks our caregivers are committed to ensuring clients stay physically and emotionally engaged, helping maintain their overall quality of life.
What is Light Housekeeping?
Light housekeeping services help ensure a clean, safe, and comfortable living environment for seniors. This includes tasks like disinfecting surfaces, cleaning kitchens and bathrooms, vacuuming, de-cluttering, and taking out the trash. We tailor our care plans to each client’s physical abilities, ensuring they get the assistance they need without losing their sense of independence.
Many clients enjoy participating in these activities alongside our caregivers, building relationships while maintaining a tidy home.
What is Respite Care?
Respite care offers temporary relief for family caregivers by transferring caregiving responsibilities to a professional caregiver or another trusted individual. Whether for a few hours or several days, respite care allows primary caregivers to take a break and recharge.
Respite care at Local Home Care can include companionship or personal care, depending on the client’s needs, giving caregivers peace of mind while ensuring their loved ones receive continuous, quality care.
What is Private Duty Nursing?
Private duty nursing involves one-on-one medical care provided by skilled professionals, such as Registered Nurses (RNs) or Licensed Practical Nurses (LPNs). Our private duty nurses offer advanced medical support in the comfort of the client’s home, assisting with more complex health needs in collaboration with the family.
This personalized approach ensures clients receive specialized care that goes beyond traditional in-home services.
What is End-of-Life Care?
We provides compassionate support for families facing the emotional and physical challenges of caring for a loved one with a limited life expectancy. We work alongside hospice providers to ensure that your loved one is cared for with dignity and comfort in their final days.
Our caregivers assist with daily tasks around the house, provide emotional support to the family, and offer companionship to the patient, helping to relieve the stress and burden during this difficult time.
What is Alzheimer’s Disease and Dementia Care?
Caring for a loved one with Alzheimer’s or dementia requires specialized support, and our caregivers are trained to provide the necessary care. We create care plans that engage clients mentally, physically, and emotionally, while also providing respite for family members.
In addition to personal care and companionship, we offer safety monitoring technologies, medication reminders, and family education to help better understand and manage dementia. Our goal is to provide compassionate, personalized care that adapts to the evolving needs of each individual.
What is the Difference Between Palliative Care and Hospice Care?
Palliative care and hospice care both aim to enhance comfort and quality of life, but they serve different purposes. Palliative care can be provided at any stage of a serious illness, often alongside treatments aimed at a cure. Its focus is on symptom management and improving quality of life.
Hospice care, on the other hand, is intended for individuals in the final stages of life, typically when life expectancy is six months or less. At this point, the focus shifts from curing the illness to providing comfort, addressing physical, emotional, and spiritual needs. Both types of care are essential in offering support to patients and their families during challenging times.
